Bran out the window and Ned's death.
I'd heard about the Red Wedding, so unfortunately it didn't shock me. I'd also finished all the books prior to Season 4 starting, so most of what has happened since I either new was coming, and in terms of stuff from the show that's not in the books, I didn't find much of it too shocking is it was so obvious a lot of it was going to happen.
In fact, I would say the most shocking thing about the show so far was the sheer aggression in the killing of Meryn Trant...now that was a pleasure to watch.

I went with Ned and Margaery's deaths. The latter moreso because I never thought they'd have the balls to do it and I figured I'd have to sit through her bullshit for a few more seasons.

Top 3 would be Red Wedding, Ned's death, and then Oberyn's. The last two, I had no idea were coming...Ned's death really set the tone for the show and Oberyn's steered us back to that place. But the Red Wedding wins simply bc I knew it was coming and who exactly was dying, yet it was still as brutal and horrific as ever. I can't even imagine what my reaction would be had I not known (I'd probably have reacted less because I wouldn't have processed/believed it). So yeah, Red Wedding takes it for surprising me in such a visceral way despite me knowing what was abotu to unfold from like, a year beforehand.

Well, The Red Wedding gave me multiple heart attacks, and also Renly, being killed all supernaturally like that made me sit up too.

Red Wedding of course, followed by Theon's castration. Oberyon's death was more soul-crushing than shocking/surprising because I felt it coming. By Season 5 and 6, I was too numbed to be too shocked by Shireen's or Margaery's death but they deserve mentions at least. I went into the first season knowing that Ned was going to die (I ended up seeing that scene on HBO before I watched the show), so that wasn't shocking either. It's not in this list, but this image was stomach churning.
