I hardly ever get properly scared by movies anymore, but there are still some scenes that fuck me up.
Basement scene, Zodiac (probably #1 honestly, and it's not even a horror film) Opening kill, Scream 'Nightmare', Rosemary's Baby Family dinner, The Texas Chain Saw Massacre Night vision, The Silence of the Lambs Tramp behind Winkie's, Mulholland Dr. Pazuzu's face, The Exorcist (only a one second flash, but it's still by far the scariest part of the film) Pretty much all of the tapes, Sinister (average horror overall, but these were effective as shit) Baby head-twist, Trainspotting

The Winkie's scene is the most unsettled I've ever been watching a scene of a film. It still works on me to a degree on my constant re-watches of the film. Honestly though, I'm pretty much one of those people who don't get scared by movies, be they horror or otherwise. I do recall when I was a kid, the kiddie vamps floating at the windows in Salem's Lot was sufficiently eerie for me. Other than that, I've got nothing. I have to disagree on the dinner scene from Texas Chainsaw. I would say that it is one of the most laughably bad & annoying moments in a shitstain of a film.

The scene in Spoorloos where the killer sits in his car in the rain and the windshield distorts his face in the creepiest way (while the hero takes the sleeping pill and scrambles like mad outside).

The hospital scene in Exorcist III has to be up there. Also it contains one of the best and most natural jump-scares I've ever seen.

The dinner scene in Eraserhead still freaks me out no matter how many times I've seen it, especially since I always put that movie on at night with all the lights out to psych myself out.

Small moment, but the part in Hereditary when Annie turns the light off in a room, sees a vision of her mother standing still in a dark corner staring at her creepily, turns the light back on and sees nothing. Not exactly a novel thing to do in a horror movie, but god that shit sends a chill down my spine every time.

Another small moment - in Carrie when Spacek returns home after the prom and walks up the stairs to the bathroom, you see her mother standing very still behind the door of the bathroom as Spacek walks in and she doesn't notice her. I didn't even notice her the first time I saw the movie because the moment is shot from kind of a distance... creepy af.
I guess there's something about uncanny stillness of characters shot from far away that always gets me (like the twins in The Shining too).
