I guess it's Downey right now, mainly because of the Marvel movies.
But I have faith in Leo wining this. Far better actor. His four year absence could have hurt him but it seems that's not the case after all.

DiCaprio, his films do a 100 million plus regardless of property or franchise. This decade only J. Edgar did less, and even then for a Oscar/critical flop, it did pretty well.

How is Scarlett Johansson even an option, and Dwayne Johnson isn't? It's Johnson btw. He brings people into stupid movies that would make less than half what they do without him, through sheer force of personality and likability. He's kind of half Scharwazneggar and half Will Smith. Without having movies as good as either of them.

The only contenders are Leonardo DiCaprio, Dwayne Johnson, and Robert Downey. Everybody else is clearly eclipsed by one of these three in every possible way. With that said, I think it's DiCaprio. There is a reason DiCaprio doesn't shy away from PG-13 action movies or R-rated comedies while Dwayne Johnson doesn't even attempt R-rated dramas and mostly only does PG-13 action movies. DiCaprio can sell anything (Inception is the highest grossing original action movie of all time at $830M, The Wolf of Wall Street is an R-rated comedy that made $400M, and The Revenant is an R-rated drama that made $530M). Johnson can only sell action movies and comedies, which are among the easiest sells of all.
Is there any way Dwayne Johnson can be added to the poll, by the way?

DiCaprio's the only one with the air of invincibility I associate with the title of "biggest movie star" (thinking of a prime Cruise or Hanks). Whatever he's in, I have full confidence it'll cut a profit.
With RDJ, the only time he's tried to sell an original film on his own name since Due Date was the poorly performing The Judge and unlike the early days of the MCU, the brand is big enough to have historic success without his name attached - back in 2011, Thor and Captain America did good but nowhere near Iron Man levels; nowadays, Captain Marvel and Black Panther can gross $1 billion.
The Rock's films always do good overseas, but factoring in the marketing and the reduced take that studios receive from overseas box office (especially in China), and I'm not sure Skyscraper or Baywatch even broke even.
Brad Pitt makes no sense to me to be on this list. He's always been tabloid fodder, sure, but as far as a box office draw, he's never been all that consistent or carried that many unexpected successes; hell, I think it's still safe to say he still currently isn't as big as Will Smith. Scarlett Johansson is a solid draw, but I can't see any case being made for her in the top 3 let alone #1. Tom Cruise feels like an oversight on this poll since his numbers recently still deserve legitimate consideration and I wonder if he gets underrated as a draw some these days because it's not as ridiculous as his peak and/or he's just been so big so now that now there's some fatigue in anointing him as the biggest movie star.

Well, I just looked it up and there is no wonder Dwayne Johnson rarely does R-rated movies (except for Baywatch, which was pre-existing property and still didn't do all that well). His lifetime gross in R-rated movies is $356M on a combined budget of $196M. That's less than break even territory. The Revenant alone grossed 50% more than all of Dwayne Johnson's R-rated movies combined.

Mike nailed it. DiCaprio is the only one who can sell any project regardless of genre or subject matter and without having to rely on previously established properties/franchises. It also helps that he's so selective with his projects, meaning his star wattage isn't diluted by appearing in too many films; every new one becomes an event.

It's Leo. The Rock is certainly a draw, but he does high concept stuff that doesn't necessarily need him to be successful. Sure, he has an established fanbase and will bring more to the table than some generic B-lister, but I think if you put another huge star like Cruise or Pitt in some of his roles the movies don't make any less money.
Leo does things nobody else can do anymore. The Revenant probably doesn't even get green lit without him, let alone with a 100+ million budget, and let alone going on to gross over 500 mil. The Wolf of Wall Street was a raunchy R-rated drama that did 400 mil.. Shutter Island was a non-high concept thriller with a confusing plot that did 300 mil.. And you can't pin those on Scorsese since he never has big hits like that without Leo. Leo takes movies that wouldn't otherwise be commercial and makes them commercial. The one exception to this might be Inception, but even that isn't really high concept (try to explain the plot in two sentences) and confused general audiences. I know Nolan is a draw too, but none of his non Batman stuff made as much as Inception. Leo's the one guy, aside from maybe Cruise, who's an automatic greenlight for anything.
I think you could maybe argue that Cruise could do this, but he hasn't put that to the test in recent years and has played it safe in standard blockbuster action movies, much like The Rock (who I don't think could open a movie outside his niche anyway). It would be interesting to see what would happen if Cruise decided to start doing mid budget prestige dramas and maybe took a year or two off in between projects to build anticipation. I would really rather see him do that since I think he's a better actor than Leo anyway.

Kind of weird that you think this is a "problem" limited to the public. Literally every section (directors, actors, craftspeople, cinephiles, critics) takes him very seriously as an actor. Odds are, even your favorite director and actor, whoever they are, take him very seriously as an actor.
